When to perform the embryo scoring for the prediction model?

The embryo observation should be performed about 46 hours after insemination of oocytes or ICSI – i.e. normally at 12.00 – 13.00 on day two. That is, , the embryo has normally left the two-cell stage and reached the four-cell stage.

If the scoring is done at an earlier time point (in the morning), there will be a great risk of  underestimation of the implantation chance.

What to record in BL, EQ, NU, FR and SY for the embryo?

BL = The number of blastomeres. Se answer above.

EQ = Regularity, i.e similarity in size between the blastomeres. The value is a classification in three steps, 0-2.
Classification:
- Value 0 if regular size of all cells in the embryo
- Value 1 if some inregularity, but in total less than 50%
- Value 2 if very inregular, more than 50%

NU = Number of nuclei found. Se answer on question below.

FR = Fragmentation. The value is a classification in 5 steps, 0-4.
Classification:
- Value 0 if no fragmentation
- Value 1 if minor fragmentation, less than 10%
- Value 2 if fragmentation is between 10% and 25%
- Value 3 if fragmentation is between 25% and 50%
- Value 4 if fragmentation is more than 50%

SY = Symmetry. The value is a classification in 3 steps of the symmetry of the cleaved embryo, 0-2.
Classification:
- Value 0 if fully symmetric cleavage
- Value 1 if slightly asymmetric cleavage
- Value 2 if totally asymmetric cleavage

How do we see and count the nucleus in the embryo?

There are two methods to count the nuclei.

If you use a normal microscope, a method is to use the mirror in the microscope to change the angel of the light at the same time as the embryo is rotated.

Another method is to use the ICSI microscope to count the nuclei.

It may be difficult to visualize the nuclei and you may need som practice. However, it is worth the effort, because failure to visualize the nuclei will lead to an underestimation of the embryo score.
